Nurse Practitioner Jobs Key Responsibilities
As a Nurse Practitioner in Palliative Care, your role will revolve around consultation, symptom management and supportive care.
- Work with an interdisciplinary team to implement patient care plans
- Clarify patient and family goals of care
- Connect patients with hospice, rehabilitation or aggressive treatment options
- Assist in diagnosis, treatment, and management of acute and chronic conditions

Nurse Practitioner Jobs Education & Certifications
- Master’s degree in Nursing from an accredited university
- Licensed to practice in the state of Illinois
- Certified as a Clinical Nurse Specialist or Nurse Practitioner
- Certification by a national body (ANCC, AANP) required
- ACHPN certification from the Hospice Palliative Credentialing Center (HPCC) preferred
- DEA license required
- Reliable transportation, valid driver’s license, and auto insurance
Experience Required
- Minimum 5 years of nursing experience
